In the current VWS-Monitor, four public values (accessibility, quality, affordability and involved society) are linked to social objectives. Examples of social objectives are optimal freedom of choice for type of childbirth and curbing rising healthcare costs. These objectives can be measured by linking them to indicators that specify what is needed to achieve these objectives goals. For example, there are indicators that provide insight into developments in major cost items, such as long-term care.

In addition to current social objectives, it is recommended to include specific goals with a more direct link to existing policy. That way, the Monitor would improve its accountability function to the Lower House.

In addition, it is recommended to systematically address the continued development of the Monitor, for example, by having all aspects of the public value clearly reflected in the social objectives. Accessibility as a public value would include financial accessibility, geographical accessibility and timeliness. Another recommendation is to add a description of how the Ministry decided to include certain social objectives and indicators in the Monitor. This information would provide insight into the considerations that were made and make it possible to weigh the information better. This would ensure that choices are not perceived as arbitrary.

Furthermore, it is important to be able to interpret the figures yielded by the indicators. This can be done by comparing the figures internationally, with a standard, or by showing the evolution of the figures of the indicators over time.

The advice was prepared by the Consortium of the State of Public Health and Health Care (consisting of: the National Institute for Public Health and the Environment (RIVM), Statistics Netherlands (CBS), the Dutch Healthcare Authority (NZa), the National Health Care Institute (ZIN), the Trimbos Institute, the Netherlands Institute for Health Services Research (NIVEL), the Netherlands Institute for Social Research (SCP) and the Netherlands Youth Institute (NJi)). The Consortium was asked to provide this advice because it has developed a website named the State of Public Health and Health Care, that is specifically aimed at supporting policy with figures.

In de huidige VWS-monitor worden vier publieke waarden (toegankelijkheid, kwaliteit, betaalbaarheid en betrokken samenleving) gekoppeld aan maatschappelijke doelstellingen. Voorbeelden van een maatschappelijke doelstelling zijn optimale keuzevrijheid voor het type bevalling en het beperken van de stijging in de zorgkosten. Deze doelstellingen zijn meetbaar gemaakt door ze te koppelen aan enkele indicatoren die aangeven wat nodig is om de doelstellingen te realiseren. Zo zijn er indicatoren die inzicht geven in de ontwikkelingen in grote kostenposten, zoals de langdurige zorg.

Geadviseerd wordt om naast de huidige maatschappelijke doelstellingen specifieke doelen met een directere link naar het bestaande beleid op te nemen. Op die manier voldoet de monitor beter aan de verantwoordingsfunctie richting de Tweede Kamer.

Daarnaast wordt aanbevolen de doorontwikkeling van de monitor systematisch aan te pakken, bijvoorbeeld door alle aspecten die bij een publieke waarde horen terug te laten komen in de maatschappelijke doelstellingen. Bij toegankelijkheid als publieke waarde zouden dan onder meer financiële toegankelijkheid, bereikbaarheid en tijdigheid horen. Een andere aanbeveling is om een beschrijving toe te voegen hoe het ministerie tot de keuze van bepaalde maatschappelijke doelstellingen en indicatoren in de monitor is gekomen. Die informatie geeft inzicht in de gemaakte afwegingen en maakt het mogelijk de verstrekte informatie beter te wegen. Dit kan voorkomen dat de gemaakte keuzes als willekeurig worden gezien.

Verder is het van belang duiding te geven bij de waarden die de indicatoren weergeven. Dat kan door de waarden te vergelijken met andere landen, met een norm, of de ontwikkeling van de waarden van de indicatoren door de tijd heen te laten zien.

Het advies is opgesteld door het Consortium van de Staat van Volksgezondheid en Zorg (bestaande uit: het Rijksinstituut voor Volksgezondheid en Milieu (RIVM), het Centraal Bureau voor de Statistiek (CBS), de Nederlandse Zorgautoriteit (NZa), het Zorginstituut Nederland (ZIN), het Trimbos-instituut, het Nederlands instituut voor onderzoek van de gezondheidszorg (NIVEL), het Sociaal en Cultureel Planbureau (SCP) en het Nederlands Jeugdinstituut (NJi)). Het Consortium is gevraagd om dit advies te geven omdat het de Staat van Volksgezondheid en Zorg onderhoudt; een website die er specifiek op is gericht om beleid met cijfers te ondersteunen.
